End of life conversations can be difficult, but coming to the end of our lives is a form of growing or transitioning; something we have been experiencing throughout life. February 23 at 6:30 pm, is an opportunity to learn and talk about what transitions, challenges, and choices there are at the end of life, such as knowing how and when to make decisions about choosing home health, palliative care or hospice care and what the differences are between them. Come learn the variations in the types of care offered in the community and at home. Learn what questions to ask and more.
